LIFE INSURANCE

MetLife - Covers one time your annual salary up to $50,000 basic life and accidental death and dismemberment (AD&D) insurance.

SUPPLEMENTAL LIFE INSURANCE

You may purchase additional supplemental life insurance through MetLife.

SHORT-TERM DISABILITY (Not Applicable to Part-Time Associates)

MetLife - This benefit pays 60% of your salary to a maximum of $3,000 per week.

LONG-TERM DISABILITY (Not Applicable to Part-Time Associates)

MetLife - In the case of a disability lasting longer than 90 days you may receive 60% of your monthly salary to a maximum benefit of $10,000 per month.

401(k)

Administered by Prudential, AMERIGROUP Corporation Retirement Savings Plan offers 12 different investment options.  The company matches 50% of the employee’s contribution up to six percent.

EMPLOYEE STOCK PURCHASE PROGRAM

The ESPP allows you to invest in AMERIGROUP Corporation by contributing 2 to 10 percent of your salary on an after-tax basis. Employees receive a 15% discount on stock purchases through the ESPP.

FLEXIBLE SPENDING ACCOUNTS

Administered by FlexAmerica, you may contribute a maximum of $3,000 per plan year to a medical account, $5,000 per plan year to a dependent care account, $2,580 for parking expenses and $1,320 for transportation expenses.

EMPLOYEE ASSISTANCE PROGRAM

This short-term resolution-oriented program covers such areas as alcohol and drug abuse, family/children problems, marital relationships, job conflict, depression, stress or legal/financial issues.

PAID ACCUMULATED LEAVE (PAL)

Exempt (exempt from overtime pay):
0 - 4 years: 20 days
5+ years: 25 days
    Non-exempt (eligible for overtime pay):
0 - 4 years: 15 days
5+ years: 20 days

PAL is paid time off from the workplace for benefit-eligible associates and is accrued at the rate of 4.62 hours per pay period for non-exempt associates and 6.15 hours per pay period for exempt associates.

VOLUNTARY SUPPLEMENTAL BENEFITS

These benefits cover short-term disability, accidents, cancer, hospital confinement and critical illness.

RELOCATION

Relocation assistance is available for eligible positions.
